Résumé

Glioblastoma represents the highest grade of brain tumors. Its growth is promoted by a strong neovascularization and by the activation of the EGFR-Ras pathway The first part shows the antiangiogenic and antitumoral effect of cartilage extract in several models of glioblastoma in mice. The study of the transcriptome of endothelial cells has permitted to propose some mechanisms of action. It was postulated that the cartilage extract could induce secretion of cytokines, which cause the generation of reactive oxygen species. These would lead to the activation of NFkB, which would induce the expression of a first network of genes involved in inflammation of endothelial growth. Co-administration of corticosteroids, to reduce tumor edema, has resulted in a loss of efficacy in vivo and should be avoided in clinical practice. The cartilage extract also overexpressed a second network of genes involved in proteolysis of the matrix, required for vascular tube stabilization. The essential role played by tissue plasminogen activator (tPA) has been demonstrated by a complete functional study of the fibrinolytic system in vivo. Finally, the formation of tumor edema, caused by that system could be controlled by co-treatment with N-actetylcysteine, which significantly increased survival in the intracerebral model of mice glioblastoma. This co-treatment would be recommended for clinical trials. The second part demonstrates that the effect of TLN-4601 relies on a chronic exposure, which guided the design of the therapeutic regimen in clinical trials. It was shown that this farnesylated dibenzodiazepine, could act on the peripheral benzodiazepine receptor (PBR) and farnesylated messengers such as Ras, Raf and Rho involved in the EGFR pathway. The accumulation of the compound in tumors, the effects on steroidogenesis and the mitochondrial depolarization could be attributed to the PBR. The induction of a network of genes responsible for catabolism of glycerol and beta oxidation of fatty acids, would remove the substrates required for synthesis of membrane phospholipids and growth of tumor cells. This metabolic change could result from the inhibition of Akt downstream of Ras. Therefore, patients with mutations of MDM2, p53 and PTEN, frequently found in GBM, would in theory not be good candidates, since they are able to activate the RAS pathway downstream of the effect of TLN. In contrast, overexpression of PBR would be an excellent criterion.
